Technical Assistance:
Technical assistance today included emergency procedures; record keeping; age appropriate crafts materials; and outdoor areas.

Comments:
An unannounced monitoring inspection was conducted today from 1pm - 3:15 pm. All classrooms in both buildings were well within ratio. The visit occurred at naptime, so I observed children resting to quiet music. I also saw the transition after nap to snack and play opportunities. Lots of choices for play observed. Both buildings were well-stocked with age appropriate learning activities. If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of a sample of children's records today, it was found that some required information was not documented. Evidence: None of the forms in the four records checked today had information regarding names of additional programs or schools that the child is concurrently attending.

Plan of Correction: This will be added to the new enrollment forms.

Standard #: 22VAC40-185-90--A
Description: Based on a review of children's records today, the written agreement between parent and center did not include all required information. Evidence: The agreement was missing the statement that the parent should notify the center within 24 hours or the next business day after any member of the child's immediate household develops a reportable communicable disease. The current statement only includes siblings.

Plan of Correction: Corrected on the computer during the inspection; will go to all parents.

Standard #: 22VAC40-185-260-A
Description: Based on a review of records today, the current fire inspection expired 6/16.

Plan of Correction: We paid for the inspection on 7/20 and have been trying to schedule an inspection.

Standard #: 22VAC40-185-270-A
Description: During today's inspection, some outdoor areas of the center were observed to not be maintained in safe /operable condition. Evidence: Near the wooden slide structure in the pea gravel, hard black plastic and soft black landscape fabric was exposed on the ground, posing a tripping hazard. There was some splintering areas observed on a wooden structure. The two outdoor fan/air conditioning units were observed to be accessible to children in the grassy area of the yard. Two barbeques, one with propane, were accessible near where children eat lunch outdoors. The cord from the table umbrella on the playground picnic table was hanging in a loop, accessible to children.

Plan of Correction: The landscape material will be removed. The wood areas will be sanded. A barrier will be put up to prevent access to the air conditioning units. The barbeques will be covered. The string on the picnic table will be made inaccessible by tying with bands to keep it high and out of reach.

Standard #: 22VAC40-185-280-B
Description: During today's inspection, a hazardous substance was observed to be in an unlocked place accessible to children in care. Evidence: A gallon sized container of cleaning fluid was observed under the sink in an unlocked cabinet in one of the children's bathrooms.

Plan of Correction: Removed during inspection. Teachers will be reminded.

Standard #: 22VAC40-185-330-B
Description: During today's inspection, an inadequate amount of resilient surfacing was observed in one specific area of the playground. Evidence: While other areas had loose fill rubber mulch and industrial resilient carpet; the area in the fall zones of the wooden slide had pea gravel, but there were some bare spots and other areas beside the slide with less than six inches of gravel today.

Plan of Correction: A parent work day is scheduled and we will sweep the pea gravel and add more if necessary.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications on-site today, the medication for Child #1 was not sent home for replacement when it was past the expiration date as indicated in the center's medication policy. Evidence: The over the counter allergy medication for Child # 1 expired 12/16.

Plan of Correction: It wll go home today and the parent will bring in a new one.
