Comments:
Licensing Inspector arrived at 10:350m and departed at 12:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 70 children in care with 13 staff present. A sample of 7 children and 7 staff records were reviewed. The center did not have any medications or children with food allergies in care at the time of inspection. Children were observed participating in music and movement and eating lunch.

Violations:
Standard #: VENDSUB-000-012-E
Description: Based on a record review the vendor failed to ensure all staff providing care to children had the required pre-service training. Evidence: Staff #5 (doh 03/15/16) and #6 (doh 10/19/10) did not have documentation of having completed the required pre-service training.

Plan of Correction: The director will send staff #5 and #6 home and have them complete the required 10 hour training.

Standard #: VENDSUB-000-031-A
Description: Based on a record review and interview the vendor failed to have all required information was included in the written emergency preparedness plan. Evidence: The vendor's emergency preparedness plan did not include reunification plans for the evacuation, shelter-in-place and lock down.

Plan of Correction: The director will contact the district manager to include all required information in the emergency plans.
