Technical Assistance:
Discussed the following requirements: reunification plan, and lockdown procedures, and children's written allergy plan.

Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 44 children directly supervised by 6 staff members. The children were engaged in various activities, to include: water play, outdoor time, infant care, hand washing, and free play. Areas of non-compliance are identified in this report. The exit interview was conducted with the Site Director. Inspection conducted from approximately 9:55am to 1:15pm. Violations:
Standard #: VENDSUB-000-004-F
Description: Based upon observations made and interview, the vendor did not post, with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children's allergies, sensitivities, and dietary restrictions. Evidence: A current list of all children's allergies, sensitivities, and dietary restrictions were not posted in the facility.

Plan of Correction: I will create the list.

Standard #: VENDSUB-000-006-B
Description: Based upon documentation reviewed, children's records were incomplete. Evidence: Three of three children's records reviewed did not have an allergy care for children with diagnosed food allergies (Child A, B, and C).

Plan of Correction: I will talk to the parents and request the information.

Standard #: VENDSUB-000-012-B
Description: Based upon documentation reviewed, staff did not complete within the first 90 days of employment or subsidy vendor approval the Virginia Preservice training for Child Care Staff. Evidence: Two of the five staff records reviewed did not contain documentation to verify the Virginia Preservice Training for Child Care Staff (Staff A and B).

Plan of Correction: Staff completed the training but I will obtain a copy of the certificates.

Standard #: VENDSUB-000-031-B-3
Description: Based upon documentation reviewed, the emergency preparedness plan did not include lockdown procedures. Evidence: The center's emergency preparedness plan did not include lockdown procedures. The plan is a template that has not been filled out

Plan of Correction: I will update the parents handbook, and notify them.
