Technical Assistance:
The center has shifted to a computerized system for tracking emergency drills. Discussed with director that computer records are acceptable if they record all they required information about the drill.

Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:15pm. There were 55 children directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed preparing for lunch consisting of quesadillas, pears, coleslaw and milk. Teachers were observed serving food, bottle feeding infants and burping infants. There were no medications at the center today. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all equipment in the center was maintained in a safe and operable condition. Evidence: A broken toy vacuum with sharp plastic exposed edges was observed in Early Learner's classroom.

Plan of Correction: Toy discarded.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Spray bottles of cleaners and disinfectants were observed in an unlocked cabinet in the Early Learner's classroom. 2. Spray bottles of cleaners and disinfectants were observed in an unlocked cabinet in the Kindergarten Connection classroom.

Plan of Correction: Teacher retraining.

Standard #: 22VAC40-185-400-B
Description: Based on direct observation, not all behavioral guidance was constructive in nature, and age and stage appropriate. Evidence: 1. While cleaning up stamps and transitioning to wash hands, Staff 1 was heard telling Child A that she would "not get lunch". 2. While transitioning from hand washing to lunch, Staff 1 was heard telling Child B that they "will not get lunch". 3. Staff 1 was then heard listing three children's names that would not receive lunch that day.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not dispose of soiled diapers in an appropriate receptacle. Evidence: 1. The diaper pail next to the changing table in the Exploration Station Classroom did not have a functional foot pedal to open the lid. 2. The rest room trash can in the Exploration Station Classroom lid has broken off from the base, rendering the foot operated pedal inoperable.

Plan of Correction: New receptacle ordered.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, one staff member did not have a central registry check completed before 30 days of employment. Evidence: Staff # 2's record shows documentation of a central registry check having been submitted in April 2017 and resubmitted in June of 2017, however no copy of the background check documenting the findings was present on the date of inspection.

Plan of Correction: Contact central registry and/or re-send.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on record review, one staff record did not have a central registry check before three years since the dates of the last central registry check. Evidence: Staff 4 has a central registry check dated 4/17/2014.

Plan of Correction: Contact central registry and /or res-send.
