Comments:
An unannounced monitoring inspection was conducted with the director beginning at 12:15 PM and ending at 3:10 PM. There were twenty-one children, ages five through thirteen years old, in care of three staff members during the inspections. The children were observed playing with blocks and manipulatives, playing board games, and educational video games. The younger school aged children were observed napping. The older school age children went on a field trip to go bowling. Hand washing procedures were observed. Five children?s records were reviewed and in compliance. Three staff records were reviewed. There were no medications on site during the inspection. The emergency escape drill log was reviewed and in compliance. The most recent emergency escape drill was documented as July 7, 2017. The most recent shelter in place was documented as June 21, 2017. The first aid kit, flashlight, and weather-band radio were observed and in compliance. Description: Based on a review of five staff records, the center failed to obtain updated background checks every three years as required. Evidence: Staff A's most recent criminal background check was dated 10/03/2013. Staff B's most recent criminal background check was dated 9/11/2013. Staff C's most recent criminal background check was dated 9/25/2013. Staff D's most recent criminal background check was dated 9/27/2013.

Plan of Correction: The center was waiting for a definitive date for when the finger printing would need to be completed. The center will complete updated criminal background checks and place them in the staff records.
