This unannounced monitoring inspection was conducted from 9:30 am until 12:30 pm. At the time of the inspection 15 children were present with eight staff. The sample size consisted of five children's records and five staff's records. Children and staff were observed during free play, transitions, departure, and behavioral management. The violations from the previous inspection were reviewed for current compliance. No repeat violations were found. The violation found during this inspection can be found on the violation notice. Standard #: 22VAC40-185-160-C Description: Based on record review and interview, the provider failed to obtain documentation of an updated TB screening two years from the date of the previous screening for one out of five records reviewed. Evidence: 1. The record for staff 2 contained documentation of a TB dated 6/29/15. 2. The director verified the TB dated 6/29/15 was the most current TB. Plan of Correction: Staff 2 is scheduled to have a TB screening on 9/18/17.
