Comments:
An unannounced monitoring inspection was conducted on 8/3/17 from 9:22am - 11:45am. During the inspection there were 122 children ages four months old to twelve years old in care with 20 staff. A tour of the facility was conducted and children were observed playing outside, participating in various classroom activities, eating lunch, and during rest period. Records were reviewed for ten children and eleven staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of ten children's records, it was determined that the provider dii not ensure that each child has a physical within one month after attendance. Evidence: 1. The record for child #9 did not contain documentation of a completed physical examination. 2. Staff #11 (Program Director) reviewed the record for child #9 and was unable to provide documentation of a completed physical for child #9 during the inspection.

Plan of Correction: The facility responded: Child #9 has attended our school for several years and recently came back to attend our summer camp program. When his original file was transferred to his summer camp file, the physician?s form was misplaced and not transferred along with the rest of the documents. A request to obtain the child?s current physical has been placed.
