22VAC40-191 Background Checks (22VAC40-191)

An unannounced inspection was conducted today in response to a self-reported incident regarding supervision of children. Based on interviews conducted and documentation reviewed, violations were cited for supervision and are identified in the Violation Notice. During inspection 78 children ( infant - 5years old) were present supervised by 12 staff. 3 staff records and 1 child record was reviewed. Standard #: 22VAC40-185-40-E
Description: Based on interviews with staff and review of the Child Supervision Record (CSR) for 7/28/2017, It was determined that the Center was not in compliance with its own policy and procedures. Evidence: 1. Staff repeatedly stated that classroom teachers did not utilize the CSR as required; it was left in the classroom and not used to establish name to face accountability during transition. 2. Inspector observed that the child left in the classroom was not listed on the CSR in sequential order.

Plan of Correction: Staff will be trained on how to properly use the Child Supervision Record.

Description: Based on interviews with staff, it was determined that staff supervising children did not ensure Children under 10 years of age were within actual sight and sound supervision of staff. Evidence: 1.Teachers' interviewed confirmed: On 7/28/2017 while in transition from the classroom to a special event a 2 year old child in care was discovered in the 2's classroom alone. After conducting multiple interviews inspector was unable determine when the child went back to the classroom or how long the child was left in the classroom.

Plan of Correction: Staff will be trained tonight on how to properly transition children.
