Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on August 14, 2017 from approximately 12:55 p.m. until 3:30 p.m Upon entrance there were 34 children and 4 staff present with children. The record review sample consisted of 5 children's records and the required documentation on site for 8 staff. Children were observed playing large group games in the gymnasium and working on art projects and playing board games.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on observation, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on August 14, 2017, there was one child (child #3) who has a diagnosed food allergies (eggs and shellfish) and there was not documentation to include a physician's information on how to address an allergic reaction.

Plan of Correction: The vendor will talk to the parent about getting the food allergy plan from the doctor.

Standard #: VENDSUB-000-012-E
Description: Based on observation, the vendor did not ensure all staff providing care to children had within 30 days of the date of employment or 90 days from subsidy vendor approval current certification in first aid and cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: During the inspection on August 14, 2017, there was not documentation of staff #4 (date of hire 6-29-2017) obtained the required training.

Plan of Correction: The vendor will have the staff member bring in the documentation of the course or will have her take the class.
