Technical Assistance:
The cleanliness and frequency of floor cleaning was discussed with the program director.

Comments:
An unannounced monitoring inspection was conducted from 10:35am and departed at 12:00pm. At the time of the tour there were 70 children in care with 14 staff present. A sample of 7 children and 7 staff records were reviewed. The center did not have any medications or children with food allergies in care at the time of inspection. Children were observed participating in music and movement and eating lunch.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review the center failed to ensure all required information was maintained in each child's record. Evidence: Child #5 (doe 07/25/16) did not have documentation of 1 of the 2 required emergency contacts.

Plan of Correction: Second emergency contact added on 08/09/2017. Going forward all new families must have a paperwork appointment to ensure all paperwork is completed before they are able to start. Two managers sign off on all files. Rechecks of all children's files will be completed by 9/30 during our semi-annual review. Any current families that do not have complete files will have 72 hours to complete or care will be suspended. Managers will check periodically for compliance. DM will check periodically on her visits as well.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center failed to ensure all requirements were met when diapering. Evidence: In the 2 year-old room there was a wrapped soiled diaper on the changing table. The foot operated disposal system was not used.

Plan of Correction: Violation was corrected on 08/09/2017. Classroom staff will be retrained on proper diapering systems completed by 08/15/2017. Management will manage correct procedures are done by regular observations and DM will monitor at visits as well.

Standard #: 22VAC40-185-560-F
Description: Based on observation the center failed to ensure all requirements were met when serving food. Evidence: The menu posted in the child check-in area listed ravioli, broccoli and tropical fruit salad. The children were observed eating chicken nuggets, corn and pears during lunch.

Plan of Correction: Current Menu was posted on 08/10/2017. Food Manager retraining will be completed by 08/16/2017. Management staff will continue to monitor food being served versus menu to ensure accuracy. If any substitutions are made, food manager will ensure that notice of substitution is posted prior to food service. DM will continue to monitor periodically on visits as well.
