Comments:
An unannounced renewal inspection was conducted today from 9:00am-12:00pm. There were 57 children (infants-5yrs.) directly supervised by 9 staff. The physical plant, 6 staff records, 5 children?s records, 2 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, singing and dancing, reading books, and eating snack. Snack served for the day was pineapples and milk. Infants were observed being held and bottle fed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on observation, the center did not maintain complete children's records. Evidence: Child A (start date: 8/7/2017) was missing an immunization record.

Plan of Correction: I will request records from parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a safe and operable condition. Evidence: 1. In the pre-k bathroom there is a cracked, loose tile and a missing tile on the bathroom floor. 2. In the pre-k room, and 3s and 4s room, two play kitchens were missing equipment , exposing holes which are potential finger entrapments. 3. In the 2s room, a play kitchen was missing a faucet and a screw head was exposed. 4. The handlebar in the older infant classroom was falling off and could potentially fall on a child. 5. In the shared bathroom between the infants B and toddler classrooms, there is insufficient lighting and causes a safety concern.

Plan of Correction: Work orders are in place. Director will expedite requests.

Description: Based on observation, a functional battery operated radio was not available. Evidence: A battery operated radio was available at the center but did not have batteries to function.

Plan of Correction: I will purchase batteries.
