Comments:
An initial study was conducted beginning at 10:00 a.m. and ending at 1:15 p.m with the director and new owner. This is an existing center that is in the process of changing ownership and operational responsibility. There were twenty-seven children in care with four staff during the walkthrough on August 9, 2017. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in safe, operable condition. Evidence: In the Two's room, the ceiling near the changing area is sagging, cracked and chipped. On the playground, the wooden hanging climbing structure is not anchored, creating an entanglement hazard. There is a broken board on the hanging structure creating a snag and scratching hazard. On the pool deck, there are three loose deck boards that create a tripping hazard. There is broken PVC that holds the stairs for the pool that creates a scratching/snag hazard. There is a broken board in the front of the pool deck that has sharp edges that is a tripping/snagging/scratching hazard. There are several screws and nails and broken pieces of wood. There is a broken piece of wood on the upper hand rail of the pool deck that creates a scratching hazard.

Plan of Correction: Contractor has been hired to fix all repairs. Pool will be closed for the season.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the resilient surfacing under playground equipment was not sufficient in fall zones at least six inches deep. Evidence: Pea gravel was measured at one to three inches in depth in front of the slide and swings. There were several areas of dirt observed near the swings.

Plan of Correction: Pea gravel will be raked to sufficient depth beneath swings and slides. Obtaining quotes to have resilient surfacing replaced with rubber mulch or pour n play.
