Comments:
Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on August 8, 2017 from 10:50 a.m. through 12:45 p.m. On this date, there were twenty (41) children in the care of seven (7) staff members. There are a total of one-hundred seven (107) enrolled in this after-school program. Five (5) children's records and four (5) staff records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were outside play, transition, hand washing/bathroom procedures, lunch and departure for a field trip. All interactions between staff and children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Description: Based on the record review, two children's records did not have a physical address listed for the second emergency contact person. Evidence: 1. Five children's records were reviewed. 2. Child A and B did not have documentation of a physical address for the second emergency contact person. 3. The director verified no physical address was documented for Child A or B.

Plan of Correction: The director will ask the parents of Child A and B to furnish documentation of a physical address for the emergency contacts.
