Comments:
An unannounced renewal inspection was completed on 8/21/17. There were 23 children present with four staff supervising. Five children's records and four staff records were reviewed. There were no medications at the site. The inspector arrived at the school at 3:00 pm and departed at 5:30 pm. The children were observed during gym times, during snack, during homework time, during table games, and during departures. There was discussion with the site director about the following: upcoming renewal of health report form, monitoring bathroom supplies in the public school to ensure there are supplies available for hand washing and toileting, method of sanitizing the tables.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to ensure that staff had a tuberculosis (TB) test at least every two years. Evidence: 1. There was documentation of a TB test in the record of staff #2 that was completed on 8/18/15.

Plan of Correction: According to the child care director, the staff person had been to get a TB test administered today which will be read for results on Wednesday, 8/23. For future, the child care director will inform site directors of updates needed with dates and will monitor the list of updates needed monthly to ensure updates of staff records are completed.

Standard #: 22VAC40-185-60-A
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that one of five children's records was complete as required. Evidence: 1. The record of child #1 was missing a complete address for one of two emergency contacts.

Plan of Correction: This was corrected during the inspection. The site director completed the address during the inspection.
