An unannounced monitoring inspection was initiated today at 12:30pm. The physical plant, 5 staff files, 5 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 92 children under the supervision of 19 staff. During the inspection, children were observed finishing their lunch, cleaning up for and resting at naptime, listening to "Moana" on tape, enjoying free play and watching a live stream of the solar eclipse. For lunch, the children were served pasta with chicken sausage and butternut squash, salad, peaches and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 4 staff certified in MAT (medication administration training). Standard #: 22VAC40-185-160-C Description: Based on file review, 3 out of 5 staff records do not have a TB test/screening completed within the past two years. Evidence 1. Staff #1 and #2's most recent TB test/screenings were last completed on 7/17/15. 2. Staff #3's most recent TB test/screening was last completed on 8/13/15. Plan of Correction: These will be completed again.
Standard #: 22VAC40-185-70-A Description: Based on file review, 5 out of 5 staff records do not have all required documentation. Evidence 1. Staff #1-#4 do not have written Information, to be kept at the center, as to whether or not they have any health problems which may interfere with fulfilling the job responsibilities. Plan of Correction: The Human Resources department is planning to include this in the position descriptions for all staff.
Standard #: 22VAC40-191-40-D-3-D Description: Based on file review, 3 out of 5 staff records do not have all required background checks completed before three years since the last checks were completed. This includes sworn disclosure statement (SDS), criminal history record check (CHR) and search of the central registry (SCR). Evidence 1. Staff #2's most recent CHR and SCR were completed on 5/16/14. 2. Staff #4's most recent SCR was completed on 10/18/13, CHR on 10/4/13 and SDS on 11/4/13. 3. Staff #5's most recent CHR was completed on 6/20/14 and SCR on 6/18/14. Plan of Correction: These will be completed again.
