Comments:
Today an unannounced 60-day visit and a self-reported visit was conducted with Interim Assistant Principal. This visit took place in the morning between 8:45am-11:15 there were 62 children with 13 staff within the supervision guidelines. The children were observed having morning free play, story-time, reading books, outdoor play on playground, computer time. Observation of the face to name transition sheet were reviewed and observed. A complete inspection of the physical plant, fire drill log, no medications on-site, and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-270-A
Description: Based on observation of the center, the following areas of the center were found not being maintained in a clean, safe and operable condition. Evidence: In two of the 5 classrooms observed, visible cracks/holes on the wall and the tile on the floor (infant classroom) had black dirt/stains on the tile/floor.

Plan of Correction: Assistant Principal stated that the maintenance supervisor will come in to fill the holes and paint.
