Technical Assistance:
We discussed the requirements for children's records. The risk assessment for a violation was provided. Also, we reviewed the requirements for sight and sound during transitions before snack and the snack nutrition requirements/menu use.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 3:15 PM to 6:35 PM. A supplemental inspection to check additional subsidy health and safety requirements was also conducted at this time. Today, there were thirty-five school age children with three to five staff. I viewed program activities, daily routines, snack, supervision, staffing, interactions with children, posted information, injury records, emergency supplies and drill records, emergency plan, agency inspection reports, five records for children, seven records for staff (training and TB screening), staff qualifications, indoor and outdoor areas. Homework time was in the cafeteria today. Hand washing routines were observed around snack time. The children enjoyed active play in the gym and outside. Standard #: 22VAC40-185-60-A
Description: Based on a review of five records for children, and interviews with administrative staff, the first date of attendance was not documented in all children's records. Evidence: The section for completing the start date by center staff was left blank for child 2, 3 and 4. Child 2 attended other programs operated by this agency and it was determined that the start date was in 2016. Administrative staff determined that child 3 began attending at the start of this school year and child 3 started attending this week.

Plan of Correction: Administrative staff will determine and add the start dates in the records for child 3, 4 and 5. The administrative staff will review all children's records and make sure the first date of attendance is written in each child's record. The assigned administrative staff will make sure the records contain all required information.
