An annual subsidy health and safety inspection (SHSI) was conducted from 11:30 a.m.-3:00 p.m. with the director. Four staff records and six children's records were reviewed. Based on observation and documentation review, two violations of the subsidy requirements was issued. A licensing monitoring inspection was also conducted (see separate report dtd 8/25/17). Standard #: VENDSUB-000-012-B Description: Based on review of four staff records, all staff did not complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment. Evidence: Staff C's (hired 5/16/17) record did not include documentation of Virginia Preservice Training. Plan of Correction: Staff C completed this course. Documentation of this completion will be located and placed in his record.
Standard #: VENDSUB-000-012-E Description: Based on review of four staff records, all staff did not hold current certification in first aid and cardiopulmonary resuscitation (CPR) within 30 days of employment. Evidence: Staff A's (hired 7/7/17) and Staff B's (hired 12/5/16) records did not include documentation of first aid/CPR certification. Plan of Correction: Staff A is scheduled to attend a first aid/CPR certification class on 9/6/17. Staff B holds current first aid/CPR certification, and this documentation will be inserted into the file.
