22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was completed on 8/23/17. There were 97 children present in seven groupings. Ten children's records and nine newly hired staff records were reviewed. Three emergency/as needed medications were observed and one daily medication was observed. The inspector arrived at the center at 7:15 am and departed at 12:30 pm. The children were observed during arrivals, during free time in the gym, during worship time, during morning snack, during free play times in classrooms, and during outside time. Several groups of children were also observed participating in Melody Makers. There was discussion with the director about the following: ensuring child #8 has a physical documented in the record by 9/15/17,

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that two of ten children's records contained complete information that is required by the Standards. Evidence: 1.The records of children #3 and #4 were missing a complete address for one of two emergency contacts.

Plan of Correction: The director corrected this during the inspection. The director obtained the addresses. The director will ensure information is complete at enrollment for future.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that hand washing was practiced as required. Evidence: 1. Two groups of children were observed during morning snack. The two groups of children were observed leaving the cafeteria after eating and going to their classrooms. Both groups of children were engaged in free play activities involving playing with each other and various toys after going into their classrooms. The children did not wash their hands with soap and water or use disposable wipes after eating as required. 2. A three year old child was observed eating lunch. The child finished eating and went directly to their cot for nap. The child did not wash their hands or use a wipe after eating.

Plan of Correction: The director will remind staff of this requirement and monitor that it is done properly in the future. The director will also review this at the staff meeting on Monday. The inspector notified the teacher of the group of children of the one child not washing their hands. The teacher had the child wash her hands when this was brought to her attention.

Standard #: 22VAC40-185-500-B
Description: REPEAT VIOLATION Based on interviews, the center failed to ensure that diapering procedures were followed. Evidence: 1. Two staff were interviewed in two different classes. One staff stated that the diaper changing table was cleaned with a cleaning agent after each diaper change. The inspector asked if this was the only step taken after a diaper change and the staff stated "yes." A second staff person stated that the diaper changing table was only sanitized after each change and soap and water was not used each time but occasionally. The Standards require the diaper changing surface to be cleaned and sanitized after each diaper change, a two step process.

Plan of Correction: The director will ensure each staff person is following the procedures that are posted at each diaper changing area. The proper steps were observed to be posted at each diaper changing area. The director will review the procedure at the staff meeting on Monday.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the center failed to ensure that no child walked around while eating. Evidence: 1. A three year old child was observed walking around after lunch while eating goldfish crackers.

Plan of Correction: The director will remind all staff of this requirement and monitor that this does not happen in the future. This will be reviewed with all staff on Monday at a staff meeting.
