An unannounced monitoring inspection was conducted today from 10:00am-12:45pm. There were 13 children (infants-5yrs.) directly supervised by 5 staff. The physical plant, 8 staff records, 5 children?s records, 1 medications, evacuation drills, injury reports,and policies were inspected. Children were observed participating in group play, reading books, and playing outside. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-A Description: Based on record review, each staff member did not have a negative tuberculosis screening. Evidence: Staff D is missing a negative tuberculosis screening. Plan of Correction: We will get the tuberculosis screening.
Standard #: 22VAC40-185-70-A Description: Based on record review, the center did not maintain complete staff records for each staff member. Evidence: Staff A is missing one reference. Staff B is missing one reference. Staff C is missing two references. Plan of Correction: We will acquire the references.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center, inside and outside, were not maintain a clean and safe and operable condition. Evidence: In room 3a, a play kitchen has a broken handle on the door. In the older classroom which is currently unused, there are 3 screws slightly protruding from the wall which pose a safety concern. Plan of Correction: We will put in for a replacement for the broken handle in the toy kitchen. We will talk to our maintenance person and get the screws taken out.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method. Evidence: In the 3a classroom, there was a cleaning product in a unlocked cabinet. In the 2a classroom, there was a cleaner in an unlocked cabinet. In the infant classroom, there was a cleaning product in an unlocked cabinet. Plan of Correction: We will do a training on cleaning products being locked.
