Discussed with administration: New requirements for Background searches and the Sworn Disclosure Statements intended usage.
An unannounced renewal inspection was conducted today. Observed - children directly supervised by - staff members. Ratios and supervision were in compliance. A sample of 10 children's records and 10 staff records were reviewed. The physical plant, medications, emergency supplies and procedures were also reviewed. The children were engaged in various activities, to include: free play, reading, art projects, infant care, feeding and diapering. Areas of non compliance are identified in this report. The exit interview was conducted with the Director. Inspection conducted 9:30 am-12:45pm. Standard #: 22VAC40-185-70-A Description: Based upon record review, required documentation was incomplete or missing. Evidence: The record for staff A is missing character reference #2. The record for staff B is missing character reference #2. The record for staff C is missing character reference #1 and #2. Plan of Correction: The center administration will request additional references ( not related) from staff and board members.
Standard #: 22VAC40-185-270-A Description: Based upon observation, areas and equipment were not maintained in safe and operable condition. Evidence: The boys bathroom on the third floor had rusted partitions in between toilet and urinal (along the bottom) and the plastic circular grate covering a drain on the floor was not in place allowing for a potential tripping hazard. In the Koala Bears classroom the triangle vinyl mats were torn on the edges and the edges were frayed. The Bluebirds classroom has a knob missing with a sharp screw expose at child height on the cabinet in the eating area. Plan of Correction: A request will be made to correct issues in the bathroom and for missing knob. Mats were disposed over during the inspection.
Standard #: 22VAC40-185-280-B Description: Based upon observation, hazardous substances were not kept in a locked place to prevent access by children in care. Evidence: There was hand sanitizer observed on the teacher counter in the Polar Bears classroom. In the Soaring Eagles classroom the cabinet marked first aid supplies contained cleaning product that was locked to prevent access to children in care. Plan of Correction: Center administration will remind staff that all cleaning supplies need to be locked.
Standard #: 22VAC40-185-430-B Description: Based upon observation, Toys or objects less than 1-1/4 inches in diameter and less than two inches in length were not kept out of reach of children under the age of three years. Materials were not age and stage appropriate for the age of children in care. Evidence: The Inch worms classroom was participating in a beach theme sensory table with marbles buried in the sand. The marbles were less than 1.25 inch in diameter and were not kept out of reach of children under the age of 3. Plan of Correction: The marbles were removed from the supervised sensory table at the time of the inspection.
Standard #: 22VAC40-185-560-F-4 Description: Based upon observation, children three years of age or younger we fed foods that were potential choking hazards. Evidence: In the Caterpillars classroom children were served pear slices and cucumbers that were not cut in a way to prevent chocking. (1/4 inch pieces) Plan of Correction: Administration stated that the food served were new menu items and would be cut to the proper size in the future. Toddler teachers cut food to proper size while inspector was in the room.
