A Subsidy Health and Safety inspection was conducted on 08.28.17 to review supplemental health and safety requirements. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Licensing Inspector arrived at 9:15am and departed at 11:00am to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 65 children in care with 16 staff present. A sample of 6 staff and 6 children's records were reviewed in addition to 3 medications. Children were observed on the playground, reading , doing art and participating in learning centers.
Standard #: VENDSUB-000-012-E Description: Based on a record review and interview with administration the vendor failed to ensure all staff providing care to children had CPR/FIrst Aid Training within 30 days of the date of employment. Evidence: Staff #4 (doh 06/21/17) did not have documentation of having a current CPR/First Aid Training. Plan of Correction: Staff #4 is scheduled to take CPR/ First Aid Training in September.
Standard #: VENDSUB-000-031-A Description: Based on a record review the center failed to ensure the center's emergency preparedness plan contained all required components. Evidence: The center's emergency preparedness plan did not include reunification plans for evacuations, shelter-in-place and lockdowns. Plan of Correction: The corporation is working in specific emergency plans for the centers that will include reunification plans.
