22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
Today's visit was a complaint investigation due to an allegation of a forbidden action. The investigation took place on 8/15/17 with an additional agency present during the investigation. During the investigation four staff member's were interviewed and center documentation regarding a bump of unknown origin recognized by a staff member on a child's head. Based on the evidence the complaint was determined to be "not valid". Violations were issued for standards regarding reporting injuries. Standard #: 22VAC40-185-550-L Complaint related: No Description: Based on the inspector's investigation during a visit to the center regarding an allegation that child received an injury resulting in a noticeable bump on the head of, the center failed to notify the parent by the end of the day of any known minor injuries. Evidence: Based on interviews with staff members including the owner, Program Director, and staff involved with the care of child #1 the day of 8/1/17 and center documentation, a bump on the head was noticed by staff member #2 and reported at approximately 11:00am to the Program Director. According to attendance records the child entered care at 8:30am and the bump was not noticed by any staff members at that time. The children had reportedly finished playing outside when staff #2 noticed the bump. The parent was not notified by the end of the program day of any known injuries after a bump on the child's head was noticed by a staff member. Plan of Correction: Five Star will ensure staff provide the proper documentation of any injury and to ensure that they are observing the children when they enter care.
