Comments:
An investigation was begun on 6/23/17 regarding self-reported incidents that occurred at the facility on 6/20/17 and 6/21/17. The investigation was continued on 7/7/17. The investigation included interviews of children and staff and a review of documentation and staff and children's records. Inappropriate interaction between a staff person and children was reported and was determined to be valid in violation of the Standards. See the Violation Notice for violations cited. Standard #: 22VAC40-185-240-B
Description: Based upon staff interview, the facility has not ensured that by the end of the first day of supervising children, staff are provided in writing the required information. Evidence: 1. According to the program director, staff 3, the 24 staff who were hired to work the summer camp program and began working with the children on 6/19/17 were not given the YMCA "blue book" in which the YMCA has the written information required by the licensing standards. 2. Staff 2 stated in an interview that he was not given the "blue book" of written policies and was told by staff 1 that he would get one.

Standard #: 22VAC40-185-340-D
Description: Based upon review of staff records and staff interview, the facility has not ensured that in each grouping of children there is at least one staff member who is program leader qualified. Evidence: 1. The facility administrator, staff 3, and staff 1 verified that on 6/22/17, staff 1 and staff 2 were on duty with a group of eight and nine year old boys and that staff 1 was scheduled as the program leader daily after 2:00 PM. The record provided for staff 1 identified staff 1 as a program leader, however the documentation of the programmatic experience in the record did not verify the required six months of full time programmatic experience. Experience was documented as tutoring and children's church with no supporting documentation of hours worked/volunteered. 2. Staff 2 is seventeen years old and, thus, is not program leader qualified. 3. Staff 2 stated in interview that, although scheduled as the two staff supervising the group of children, neither staff 1 nor staff 2 was a program leader.

Standard #: 22VAC40-185-400-A
Description: Based upon child and staff interviews, the facility has not ensured that in order to promote each child's physical and emotional well-being and growth, staff interact with the children to provide needed help, comfort and support. Evidence: 1. Interviews with children revealed that staff 2 hit child 1 and child 2 on the back of their heads as they were in line entering the building from outside. 2. Staff interview revealed that staff 2 was observed tapping children on their heads or arms to get their attention.
