22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 10:30 a.m.-1:00 p.m. with the director. 54 children, ranging in age from 4 months through 4 years old, were under the care and supervision of eight staff. Children were observed listening to a readaloud, playing with toys in various centers, eating lunch, and napping. Lunch served today was pasta with meat sauce, diced carrots, mandarin oranges, and milk. Required information was posted. Both the fire and health inspections are current. The last emergency escape drill was 7/19/17. There were no new medications to review. Five children's records and three staff records were reviewed. An annual subsidy health and safety inspection was also conducted (see separate report dtd 8/28/17). Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A's record did not have documentation of a physical. Plan of Correction: The parents are aware of this required health documentation. The director will request this physical from them again today.
Standard #: 22VAC40-185-160-A Description: Based on review of three staff records, the center did not have documentation of a tuberculosis (TB) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A's (hired 3/8/17) TB test was dated 4/10/17. Staff B's (hired 9/10/15) TB test was dated 10/10/15. Plan of Correction: The director will require that newly hired staff must obtain TB tests or screening in the first two weeks during orientation before they may be allowed to begin working at the center.
