Technical Assistance:
Discussed the recent communication regarding fingerprint based background checks for child day programs. Anyone who has lived in another state in the past five years is required to request a check of the child abuse and neglect registry for each state they have lived in the past five years. New applicants and agents for licensure, approval, and registration must request an out of state check when they initially apply for a license, approval, or registration.

The licensing inspector conducted an unannounced renewal inspection Monday, August 28, 2017 from 11:15am to 2:00pm. The infants were observed playing on the floor with their teachers, resting and being fed. The toddlers were observed resting on mats. Lunch today consisted of chicken nuggets, green beans, peaches and milk. The menu was posted. All classrooms and playgrounds were inspected today. Five children?s records and five employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/14/2017 Last Shelter-in-place drill: 05/25/2017 Last fire inspection: 08/28/2017 Last health inspection: 06/30/2017 Total enrollment today: 26 Today, the following child to staff ratios were observed: Infant I (6 weeks to 12 months) ?11:3 Infant II (12-14 months) ?5:2 Toddler I (15-17 months)-6:2 Toddler II (18-24 months)- 4:1 School Age ?not present The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on September 22, 2017 or the Department cannot renew the license. Description: Based on a review of children's records on 08/28/2017, the center did not ensure that two of five records reviewed contained documentation of the viewing of the children's proof of the child's identity and age. EVIDENCE: 1. The record of Child #2, who started at the center on 05/08/2017, did not contain documentation of the viewing of the children's proof of the child's identity and age. 2.. The record of Child #4, who started at the center on 02/212017, did not contain documentation of the viewing of the children's proof of the child's identity and age.

Plan of Correction: Will check with the person-in-charge to see if she viewed it. Will let parents know that they need to bring birth certificate to center as soon as possible.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 08/28/2017, the center did not ensure that over-the-counter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: 1. Child # 4's container of Aquaphor was observed in the infant classroom. It expired 10/2015. 2. Child #5's tube of Desitin was observed in one of the Toddler bathrooms. It expired 3/2017.

Plan of Correction: Corrected on site. Disposed of products during inspection. Going to check all new containers that come in.
