Comments:
An announced initial inspection was conducted on September 1, 2017. Eight classrooms, five rest rooms and an outdoor area were inspected to be used for the children in care. Facility will provide care for children ages infancy through twelve years old. The following items were reviewed as a part of the inspection: health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. The center will be administering medication and offering transportation services. The inspection will be reviewed with the Licensing Administrator regarding issuance of a conditional license.

Standard #: 22VAC40-185-40-K
Description: Based on review, the applicant did not ensure the center shall develop written playground safety procedures which shall include: Provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children. Evidence: During the inspection conducted on September 1, 2017, the playground safety procedures did not include staff in strategic locations, scanning play activities and circulating among children.

Plan of Correction: The owner is currently revising the plan to include all requirements.

Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the applicant did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection conducted on September 1, 2017, in the School Age classroom there was a six foot bookcase that posed as a potential tipping hazard and in the Two Year Old Classroom there was a three foot bookcase that posed as a potential tipping hazard.

Plan of Correction: Owner contacted maintenance personnel during the inspection and the bookcases will be secured.

Standard #: 22VAC40-185-290-3
Description: Based on observation and inspection of the facility, the applicant did not ensure that electrical outlets shall have protective covers. Evidence: During the inspection conducted on September 1, 2017, the door to the staff lounge was open and there were two surge protectors with five uncovered electrical outlets and three uncovered outlets in the surge protector in the Three Year Old Classroom.

Plan of Correction: Outlets will be covered.

Standard #: 22VAC40-185-320-B
Description: Based on observation and inspection of the facility, the applicant did not ensure that each restroom area provided for children was equipped with all necessary supplies. Evidence: There was no soap in the restroom connected to the Infant and Toddler Classroom.

Plan of Correction: Soap is now in the restroom area.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the facility, the applicant did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: During the inspection conducted on September 1, 2017, the following was observed on the Preschool/School Age Playground: 1. The height of the base of the straight yellow slide measured at approximately 5'4" and required a 9'4" fall zone. The fall zone at the end of the slide chute was 6'2". 2. The height of the base of the curvy yellow slide measured at approximately 6'3" and required a 10'3" fall zone. The fall zone at the end of the slide chute was 6'9". 3. The height of the base of the double yellow slide measured at approximately 4'4" and required a 8'4" fall zone. The fall zone at the end of the slide chute was 6'3".

Plan of Correction: Children will not be allowed to use that equipment until approval is obtained.

Standard #: 22VAC40-185-500-B
Description: Based on observation and inspection of the facility, the applicant did not ensure that the diapering area was equipped with all necessary supplies. Evidence: During the inspection conducted on September 1, 2017, there were no gloves in the diapering changing area in the Walking Infants classroom.

Plan of Correction: Gloves were added to the diaper changing area.
