Comments:
An unannounced monitoring inspection was conducted on September 6, 2017. Upon arrival to the facility, there were 12 children present with four staff members. Additional staff were on site for administrative and support purposes. The licensing inspector reviewed supervision, staff qualifications and trainings, nutrition, required postings, the physical plant and administration. The center is not administering medication at this time. The children were napping , coloring and freely playing during the inspection. Exit interview conducted with the Program Director.

Standard #: 22VAC40-185-150-B
Description: Based on 1 of 5 records reviewed, the licensee did not ensure the physical examination was signed by a physician, his designee, or an official of a local health department. Evidence : During the inspection conducted on September 6, 2017, child #3 physical was not signed by a physician, his designee, or an official of a local health department.

Standard #: 22VAC40-185-160-C
Description: Based on 1 of 6 staff records reviewed, the licensee did not obtain documentation of a negative tuberculosis screening at least every two years. Evidence: During the inspection conducted on September 6, 2017, staff #8, last T.B. test was June 8, 2015.

Plan of Correction: The program director will not allow staff to come back until T.B. test is done.

Standard #: 22VAC40-185-40-J
Description: Based on interview and documentation, the licensee did not ensure injury prevention procedures was updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: During inspection on September 6, 2017, the injury prevention plan was not updated annually.

Plan of Correction: The program director will have it updated in the next 30 days.

Description: Based on 1 of 5 records reviewed, the licensee did not ensure the record contained all required documents. Evidence: During the inspection conducted on September 6, 2017, child #5 did not have one parent's work number.

Plan of Correction: The program director corrected while licensing inspectors were present.

Standard #: 22VAC40-185-70-A
Description: Based on 1 of 6 records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: During the inspection conducted on September 6, 2017, for Staff #2 did not include emergency contact.

Plan of Correction: The program director corrected while inspector was present.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure the outside , was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on September 6, 2017, the following was observed - In the infant room there was baseboard peeling off the wall. - In the hallway by the school age room there was a blue container cracked with pinching points. - Outside area there were large branches and trash scattered on the play area, a blue trike no pedals, handle broken on pink trike and metal chair had four rusted raised bolts.

Plan of Correction: - The program director will have the maintenance person to fix the baseboard. - The bucket was removed while inspectors were present. - The trike and chair was removed. The program director will have the landscaping company to remove the branch.

Standard #: 22VAC40-185-280-G
Description: Based on observation, the licensee did not ensure hazardous substances that are not kept in original containers, clearly indicate their contents and shall not resemble food or beverage containers. Evidence: During the inspection conducted on September 6, 2017, in the school age area there was a spray bottle of yellow substance that was not label with the content.

Plan of Correction: The room has nit been used today. Room will be cleared before children arrive.

Standard #: 22VAC40-185-440-B
Description: Based on observation, the licensee did not ensure cribs were identified for use by a specific child. Evidence: During the inspection conducted on September 6, 2017, one crib was not labeled for child #5.

Plan of Correction: The program director had staff label the crib when inspectors were present. Today was child #5 first day.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the licensee did not ensure there was a working battery operated radio. Evidence: During the inspection conducted on September 6, 2017, the licensee did not have batteries for the radio.

Plan of Correction: The program director will get batteries for the radio.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure emergency evacuation and shelter-in-place procedures/maps were posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection on September 6, 2017, there was not a posted map for shelter in place and there was not posted procedures for evacuation and shelter in place.

Plan of Correction: The program director will have the map and procedures posted in 30 days.
