A Subsidy Health and Safety inspection was conducted on 8/29/17 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted from 9:00am-4:00pm. At the time of entrance, 56 children were in care with 7 staff members present. The sample size consisted of 5 children?s records and 6 new staff records. Children were observed in teacher and self-directed activities. Center management report that there is not any medication being administered at the time of this inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-004-F Description: Based on observation and interview the vendor failed to post with parental approval or keep in a way that is immediately accessible to to staff, a current list of all children's allergies, sensitivities and dietary restrictions. Evidence: 1 - The records for children 2, 3, 4 and 5 contain documentation of food allergies however, there is not a listing of the children's dietary restrictions immediately accessible to staff. 2 - Staff 6 confirmed stated that there is not a list of allergies available. Plan of Correction: Center management will create and implement a list of allergies that is readily accessible to staff.
Standard #: VENDSUB-000-031-B-1 Description: Based on review of procedures and interview the vendor failed to ensure that emergency evacuation plans include all required information. Evidence: 1 - The vendor does not have a written reunification plan in the event of evacuation from the center. 2 - The vendor does not have a written reunification plan in the event of a shelter-in-place at the center. 3 - The vendor does not have a written reunification plan in the event of a lockdown at the center. 4 - Staff 6 confirmed that there are any written reunification plan available for review. Plan of Correction: Center management will create, write and implement reunification plans for emergency evacuation, shelter-in-place and lockdown situations.
