Comments:
An unannounced monitoring inspection was conducted on September 7, 2017. Six staff were present with 35 children upon arrival to the facility. Additional staff were on site for administrative and support purposes. Children were observed in circle time, doing table top activities that included cutting, scooping, gluing and sorting. The children were also observed having snack which consisted of bananas, apple juice and crackers. Five children's records and six staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, nutrition, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, procedures and postings, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. There are no medications administered in the center at this time.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, in four of six staff records reviewed, the licensee did not ensure that staff resubmit TB test results every two years. Evidence: During the inspection conducted on September 7, 2017, the TB test results for the following staff had expired and were completed on the dates indicated: Staff #1-6/3/15; Staff #2-7/11/15; Staff #3-7/17/15; and Staff #4-7/10/15.

Plan of Correction: We will get the TB screenings/test completed within the next 14 days.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in five of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on September 7, 2017, all five children's records did not include documentation of the names of any additional schools or programs that the child is concurrently attending and the grade or class level. The records for Child #4 and Child #5 also did not include the name of any previous child day care and schools attended by the child.

Plan of Correction: The records for Child #4 and Child #5 will be corrected within one business day. The documentation regarding additional schools will be added to registration software.

Standard #: 22VAC40-185-500-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that the diapering surface shall be used only for diapering or cleaning children. Evidence: During the inspection conducted on September 7, 2017, the licensing inspector observed the tray to a high chair on the diapering surface.

Plan of Correction: The tray was removed while the inspector was present.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection conducted on September 7, 2017, the posted maps did not include shelter-in-place locations.

Plan of Correction: The map will be updated.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, in one of six staff records reviewed, the licensee did not deny continued employment to any employee when the licensed welfare agency did not have a central registry finding within 30 days of employment or volunteer service. Evidence: During the inspection conducted on September 7, 2017, there were no results of a central registry finding in the record for Staff #6 and the staff person had not been denied continued employment. The staff was observed working on the date of the inspection.

Plan of Correction: It was submitted and the original has been misplaced. If not found within 24 hours it will be resubmitted within one business day.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on September 7, 2017, the facility had the findings from the inspection conducted on March 3, 2017 posted but the most recent inspection for the facility was conducted on August 2, 2017,

Plan of Correction: The most recent inspection will be posted today.
