The licensing inspector completed a scheduled modification study on Wednesday, September 6, 2017 from 10:45 AM to 12:15 PM. This inspection was done as a result of a modification request received by the Department. Some but not all standards were checked during the inspection. The floor plan for the building was provided. Measurements of the classroom were taken during the inspection. There were sufficient furniture and age appropriate materials available for the anticipated number of children in care. All areas of the building along with classrooms, hallways, bathrooms, storage closets, conference room and outside play area were checked. There are adequate space and toileting facilities to accommodate the program's request. Policies and procedures were reviewed and discussed during the inspection. Standard #: 22VAC40-185-270-A Description: Based on observations, the facility did not ensure all areas were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed the following on the outside grassy play area: 1. A basketball goal contained torn netting that hung down, creating a head entrapment hazard. 2. A section (two feet by one foot) of potentially hazardous wild mushrooms were observed in the play area to be used by the children. Plan of Correction: Both the netting and the wild mushrooms were removed from the play area.
Standard #: 22VAC40-185-290-3 Description: Based on observation and interview, the facility did not ensure all electrical outlets contained protective covers.
During the inspection, the licensing inspector observed two electrical outlets located in both the male and female bathrooms that did not contain electrical covers. A staff member stated additional electrical outlets needed to be purchased.
Plan of Correction: We will buy the covers and place them in the electrical outlets.
