Comments:
A Subsidy Health and Safety inspection was conducted on September 7th 2017 to review supplemental health and safety requirements. Today, I met with the Staff of the County After-School Activities (CASA) program held on site at Kenneth Culbert Elementary school located in the town of Hamilton, Virginia. At the start of this unannounced inspection there were a total of 26 children with 2 staff; a good ratio for this one group. A total of 5 children?s files and 1 medication were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review, a child with diagnosed food allergies is not listed on the allergy list. Evidence: Child#3 physical record shows that the child has a diagnosed food allergy and was not listed on the allergy list.

Plan of Correction: I will talk to the parent about the allergy, the forms and everything that is required.
