An unannounced renewal inspection was conducted on September 8, 2017. The owner, director and manager were present for the inspection. Eight classrooms were observed with the ratio and activities noted. These included: Room 1: 11 two year old children with 3 staff. Children were finishing their lunch. Room 2: 4 three year old children with 1 staff. Children were playing with toys. Room 3: 8 infants with 2 staff. Infants were sleeping. Room 4: 3 one year old children with 1 staff. Children were playing with toys. Room 5: 11 four year old children with 2 staff. Children were listening to a story. Room 6: 5 two year old children with 1 staff. Children were listening to a story. Room 7: 5 three year old children with 1 staff. Children were listening to a story. Room 8: 6 two year old children with 2 staff. Children were listening to a story. Seven children's files and 7 staff files were reviewed. There were no medications being administered. Evacuation and shelter drills had been documented. Inspection began: 10:20 Inspection ended:
Standard #: 22VAC40-185-160-C Description: Based on a review of 7 staff records, the center had not obtained results of a follow-up TB screening/test within 2 years of a previous screening/test. Evidence: The most recent TB screening/test for Staff # 7 was dated 1/30/15, Plan of Correction: The screening/test will be updated by September 12.
