Standard #: 22VAC40-185-340-D Description: Based upon review of video and staff interviews, the facility has not ensured that in each grouping of children there is at least one staff member who meets the qualifications of a program leader and such program leader supervises no more than two aides. Evidence: 1. Based upon staff interview, on 7/17/17 between 3:35 and 4:00 PM, there was a grouping of twenty-three children (five children aged five and six years and eighteen children aged nine years and older) at the swimming pool. Staff 1 was identified by administrative staff and staff 1 as the program leader of the group, supervising three staff aides. The administrator and staff 1, 2, 3 and 4 confirmed this staffing pattern. 2. Review of the provided facility video confirmed staff on duty between 3:35 PM and 4:00 PM on 7/17/17 were staff 1, 2, 3 and 4. Plan of Correction: Not available online. Contact Inspector for more information.
