VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
VENDSUB-000 SPECIAL CARE PROVISIONS AND EMERGENCIES

Comments:
Health and Safety inspection was conducted on 9/11/17, from 9am through 11:45am to review supplemental health and safety requirements. There were 12 children in care with three staff. Children present ranged in age from 2years to four years old. Children were observed in teacher and self-directed activities including lessons, restroom routines, outside play and lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-B
Description: Based on record review and interview the vendor failed to ensure that staff complete required Virginia Preservice Training for Child Care Staff. 1 - Staff 1 and 6 does not have documentation of completing the required preservice training. a. Staff 1 has a documented date of hire as 5/10/17. 2 - Staff 1 stated that she has not completed the required training.

Plan of Correction: Going forward center management will ensure that all staff complete the required training within the allotted time frames.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review and interview the vendor does not have documentation that all staff providing care to children have within 30 days of date of employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1 - The record for staff 4 does not contain documentation of current CPR training. a. The CPR documentation on file for staff 4 state that the certification training will expire 8/2017. 2 - Staff 1 confirmed that the CPR training documentation indicates that the certification has expired.

Plan of Correction: Going forward, center management will ensure that all staff receive the required first aid and CPR training within the allotted time frames.

Description: Based on record review and interview the vendor does not have documentation that all staff providing care to children have within 30 days of date of employment current certification in first aid appropriate to the age of children in care. Evidence: 1 - The record for staff 4 does not contain documentation of current first aid training. a. The first aid documentation on file for staff 4 state that the certification training will expire 8/2017. 2 - Staff 1 confirmed that the first aid training documentation indicates that the certification has expired.

Plan of Correction: Going forward, center management will ensure that all staff receive the required first aid and CPR training within the allotted time frames.

Description: Based on policy review and interview the center does not have a written evacuation reunification plan. Evidence: Staff 1 stated that the center does not have a written reunification plan.

Plan of Correction: Center management will create, write and implement and train all staff in the center's procedures for reunification after an emergency evacuation.

Standard #: VENDSUB-000-031-B-2
Description: Based on policy review and interview the center does not have a written shelter-in-place reunification plan. Evidence: Staff 1 stated that the center does not have a written reunification plan.

Plan of Correction: Center management will create, write and implement and train all staff in the center's procedures for reunification after an emergency shelter-in-place.

Standard #: VENDSUB-000-031-B-3
Description: Based on policy review and interview the center does not have a written lockdown reunification plan. Evidence: Staff 1 stated that the center does not have a written reunification plan.

Plan of Correction: Center management will create, write and implement and train all staff in the center's procedures for reunification after an emergency lockdown.
