Today's visit was a routine monitoring inspection. The inspection began at 9:45am and concluded at 2:20pm. There were sixteen children in care and five direct care staff. Additional support staff were also present. during the inspection activities were observed in the multipurpose room downstairs, lunch was observed, five out of thirteen staff records were reviewed, five out of sixty-seven children's records were reviewed, nap time was observed, emergency supplies were viewed, a sample of medications were viewed along with authorizations, and some policies and procedures were reviewed. Standard #: 22VAC40-185-140-A Description: Based on the inspector's review of a sample of five out of sixty-seven children's records, the center failed to ensure a record included a physical exam within the first thirty days of attendance. Evidence: Child #4 has a documented start date of 6/6/16 and the child's record does not include a physical exam to date. Plan of Correction: The parents will be contacted and a request to provide a physical exam will be made.
