Thank you for your time and assistance with the unannounced renewal inspection conducted today, September 12, 2017. The on-site visit was from 11:00 a.m. to 3:20 p.m. On this date, at the time of arrival, there were 44 children in three (3) preschool classrooms and in the care of seven (7) staff members. There were a total of 87 children enrolled in the preschool and before and after-school programs. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Five (5) staff members' records and five (5) children's records were reviewed. The medication and medication paperwork were reviewed for one (1) child. There were seven (7) staff members and four (4) children interviewed. Some of the items reviewed and observed included: required posted information, first aid kit and emergency supplies, updated emergency preparedness plan, injury reports, emergency evacuation drills, outdoor play, transition time, learning activities, story time, P.E. class, language art activities, lunch brought from home, and nap time. Standard #: 22VAC40-185-160-A Description: Based on review of five staff records and verification from staff, the center failed to obtain a negative tuberculosis (TB) screening within 21 days of hire for one staff member. Evidence: 1. The negative TB screening for Staff Member B was dated 08/24/2017. Date of hire was 06/12/2017. 2. Staff Member #1 confirmed the staff member was late in getting her TB screening. Plan of Correction: This was an oversight and the administration will ensure that from now on all new staff obtain a TB screening within the first 21 says of hire.
