Comments:
An unannounced monitoring inspection was conducted on Tuesday, September 12, 2017 from approximately 10:00am until 4:00pm. The following classrooms were observed: Infants: eight infants with two staff: One staff person was outside swinging four infants. The infants that were inside were either sleeping and/or receiving individualized attention. Toddlers: ten children with two staff: The children were outside playing. They were also observed preparing for lunch and nap. Two's : eleven children with two staff : The children were outside playing, Once they returned to the classroom, the staff began preparing the children for lunch and nap. Three's: fifteen children with two staff: The children were having circle time singing songs and taping to the beat of the music. Later they worked on making the letter "A" out of popsicle sticks, built with blocks and read books. Pre-K: nine children with one staff: The children were practicing writing the letters "Ff" and "Ll". They also were building with blocks and legos. The last practice evacuation drill was conducted 8/4/17. Ten children's records and ten staff records were inspected.

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility, the center did not ensure all areas, inside and outside, were maintained in a safe and operable condition. Evidence: There are several areas on the preschool playground that the black landscaping material has lifted above the mulch posing a tripping hazard.

Plan of Correction: Per Center: "The material has been removed."

Standard #: 22VAC40-185-500-A
Description: Based on observations, the center did not ensure handwashing procedures were followed. Evidence: 1. The licensing inspector observed three children that did not wash their hands after diapering/toileting in the Toddler Classroom. 2. The licensing inspector observed Staff # 3, changing children's diapers in the toddler classroom. This staff person was wearing disposable gloves that were not changed after diaper changing each of the three children. The staff person also did not wash her hands after each diaper was changed.

Plan of Correction: Per Center: "Staff will be retrained today."

Standard #: 22VAC40-185-500-B
Description: Based on observations, the center did not ensure soiled diapers were disposed of according to the Child Day Care licensing standards and that the diapering surface was cleaned and sanitized after each use. Evidence: 1. The diapering surface in the Toddler Classroom was not cleaned and sanitized after each use. 2. The staff person changing diapers opened the exterior door with her gloved hand to dispose a soiled diaper. The foot-operated storage system was outside of the classroom.

Plan of Correction: Per Center: "Staff will be retained today on proper cleaning of the diapering surface and disposal of soiled diapers."
