22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not have complete information. Evidence: 1. Child A and Child B were missing the work phone numbers for both parents listed. Plan of Correction: I will ask the parent's for the information today.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain equipment outside in a safe condition. Evidence: 1. There is a blue swing that the bar has slipped through and is coming out of the seat and not level with the other side. Plan of Correction: I removed the whole structure from the playground immediately and will have our maintenance person look at it to see if it can be repaired or it will be disposed of.
Standard #: 22VAC40-185-520-C Description: Based on review of diaper ointments and creams, the center did not obtain written authorization from the parent to apply diaper ointment or cream. Evidence: 1. Child A had a diaper ointment or cream labeled with the child's name on it, but did not have written authorization from the parent in the Toddler Room. 2. Six children had diaper ointment or cream labeled with the child's name on it, but did not have written authorization from the parent in the Two Year old Classroom. Plan of Correction: I will ask the parents to fill out the form or return the diaper ointments or cream.
