An unannounced investigation was initiated today following a self-reported incident at the center. A joint investigation was conducted. Records were reviewed and staff were interviewed on 8/24 and 8/25. The following standards were examined. 400 B: Behavioral guidance 410 1. Forbidden actions. Following careful review of available information, a preponderance of evidence was not gathered in order to validate the allegation of a forbidden action. Therefore, this is being deemed invalid. There was, however, a preponderance of evidence to validate the incident of behavioral guidance that was not constructive in nature, age and/or stage appropriate. A Violation Notice with a Plan of Correction was issued to the center. If you have any questions, please contact me at email@example.com
