Discussed with Director that staff personal items need to be labeled as best practice regarding items found that could potentially be a safety hazard to kids with allergies.
Today an unannounced monitoring visit was conducted in the morning, between the hours of 10:15am and noon. There were six classrooms observed with a total of 52 children and 10 staff directly supervising children. The children were observed, eating morning snack, outdoor play, story time, music class, self portraits, transitions to lunch and nap time. Snack today consisted of crackers and fruit cups and lunch Teriyaki chicken, rice, vegetables and pears. Standard #: 22VAC40-185-160-C Description: Based on record review, staff TB screenings were found not updated 2 years from the date of the last screening. Evidence: Staff#1 and Staff#2 did not have updated TB screening forms, the last forms expired on 05/28/17 Plan of Correction: We will have it done by the end of the week.
Standard #: 22VAC40-185-270-A Description: Based on observation of the physical plant, the following areas of the center were not found to be maintained in a clean, safe operable condition. Evidence: Two of six classrooms had chipped or peeling paint along the walls. Plan of Correction: We will repaint areas that need to be repainted.
Standard #: 22VAC40-185-510-D Description: Based on record review and observation the medication authorization form was found beyond the effective date Evidence: The medication authorization form for child #1 expired on 04/04/17 Plan of Correction: We will have mom complete a 14 day authorization form today, and we will have a long term authorization form completed within those 14 days.
Standard #: 22VAC40-185-560-G Description: Based on observation, food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence: Food was found in a cubby without name and date. Plan of Correction: Corrected during the inspection, food was labeled and dated during the inspection.
