An unannounced monitoring inspection was conducted today from 7:15am-8:30am. At the time of inspection, 32 school age children were directly supervised by 3 staff and ratios were in compliance. The sample size consisted of 5 children records and 3 staff records including: First Aid, CPR certification, and Pre-Service Training. Children were observed during arrival, engaged in group games at the tabletops, and during the transition to outdoor play time. The center was organized and clean. Staff interactions with children were positive. Area of non-compliance is identified in the Violation Notice. Standard #: 22VAC40-185-130-A Description: Based on review of children's records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child could attend the center. Evidence: Child #1 did not have record of her immunizations on file. Plan of Correction: We will get copies of the records from the school office.
