Standard #: 22VAC40-185-140-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to physicals. Evidence: Child record #3, enrollment date 8/8/2017, did not have documentation of a physical as required. The allowed 30 day time frame to obtain one had expired.

Plan of Correction: The parent will be notified as to the need of a physical. If the child returns to care then the physical will be required to be submitted on that day. Administrative personnel will continue to review files for compliance.

Standard #: 22VAC40-185-60-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to phone numbers and emergency contact information. Evidence: Child record #4, enrollment date 8/23/2017, did not have any emergency contacts listed as required. Also, there were no parental work phone numbers listed as required.

Plan of Correction: This information will be obtained from the parents and placed within the record as required. Upon enrollment all documents will be assessed for completion to maintain future compliance.

Standard #: 22VAC40-185-520-B
Description: Based on review of rooms used by the children the facility failed to maintain compliance with this standard regarding inaccessibility of sunscreen. Evidence: In the 2 year old classroom, Panda 1, there were containers of sunscreen accessible to the children. These containers were in the unlocked cabinet under the counter, beside the bathroom, at floor level.

Plan of Correction: The cabinet was locked upon the children re-entering the classroom. Staff will be reminded to either keep the cabinet locked or move the containers to another area that is always inaccessible to the children while in care.

Standard #: 22VAC40-185-520-C
Description: Based on review of rooms used by the children the facility failed to maintain compliance with this standard regarding inaccessibility of diaper ointment. Evidence: In the 2 year old classroom, Panda 1, there were containers of diaper ointment accessible to the children. These containers were in the unlocked cabinet under the counter, beside the bathroom, at floor level.

Plan of Correction: The cabinet was locked upon the children re-entering the classroom. Staff will be reminded to either keep the cabinet locked or move the containers to another area that is always inaccessible to the children while in care.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of staff files the facility failed to maintain compliance with this standard as required relating to Criminal Record Checks. Evidence: Staff record #3 did not have a renewed Criminal Record Check as required. The one on file had expired as of 9/30/2016. These are to be renewed every three (3) years.

Plan of Correction: Administration immediately sent off a request electronically during the inspection. All files will be reviewed for compliance by administrative staff to ensure future compliance. The system we use for reminders of needed updates will be reviewed to ensure the dates are correct and that the system is functioning correctly as well.
