An unannounced monitoring inspection and a Subsidy Health and Safety inspection were conducted today from 7:15am-8:45am. The Subsidy Health and Safety inspection was to review supplemental health and safety requirements. There were 18 school age children directly supervised by 3 staff. The physical plant, 3 staff records, 5 children?s records and 1 medication were inspected. Children were observed participating in group play, and playing with blocks. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on review of 5 records, the center did not maintain complete records for each enrolled child. Evidence: 1) Child A is missing two parent work phone numbers. 2) Child B is missing one parent work phone number. 3) Child C is missing one parent work phone number. 4) Child D is missing an address for one emergency contact. Plan of Correction: We will follow up with parents.
Standard #: 22VAC40-185-510-C Description: Based on review of medication, the center did not obtain both parent and physician authorization for a long term prescription. Evidence: Child D has a medication on site which does not have parent and physician authorization. Plan of Correction: Parent has the form. We will follow up to get the documentation.
