Technical Assistance:
Consultation provided on shelter in place drills, attendance log.

Comments:
An unannounced renewal study was conducted with the director, who was present with one teacher and volunteer and five preschoolers. The children were engaged in group activities during the walkthrough. The center does not administer medication. Four staff files and five children's files were reviewed today. Documentation of two injuries/accidents were reviewed as well. Arrival time: 10:15 am Departure time: 11: 50 am

Standard #: 22VAC40-185-160-C
Description: Based on a review of records, it was determined that one staff did not have a repeat tuberculosis test/screening every two years. Evidence: Staff B's tuberculosis test was dated 9/17/15.

Plan of Correction: We will have staff B obtain a new tuberculosis test/screening.

Standard #: 22VAC40-185-330-B
Description: Based on an inspection of the playground, it was determined that the center has not maintained at least 6 inches of resilient surfacing around the play equipment. Evidence: The mulch underneath and around the yellow and blue rocker was compact, not resilient and weeds had started to push through the mulch.

Plan of Correction: We will place that rocker off limits.

Standard #: 22VAC40-185-550-D
Description: Based on a review of the shelter in place drills, it was determined taht the center has not been conducting at least two shelter in place practice drills per year. Evidence: The last shelter in place drill was on 6/9/16.

Plan of Correction: We will do a shelter in place drill this month.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of records and an interview with the director, it was determined that one staff does not have a central registry finding on file within 30 days of employment/volunteer. Evidence: Staff A was hired on 7/10/17 and does not have a central registry finding on file.

Plan of Correction: We mailed for that background check on 8/31/17.
