Comments:
An unannounced inspection was conducted in the center today. At the time of entrance there were 2 staff present with 3 preschool age children. The sample size consisted of 5 children's records and 6 staff record. Children were observed during reading time, eating lunch and during their designated rest period. No medications are being administered. The inspection began at approximately 11:50 a.m. and ended at 2:45 p.m. Please contact the Licensing Inspector at (757) 404-0568 with any questions.

Standard #: 22VAC40-185-60-A
Description: Based on record review and staff interviews, the center did not ensure all children's records were maintained and kept at the center containing all required information. Evidence: During the inspection on September 18, 2017, it was determined three out of five children's records were incomplete; 1) The record for child #1 did not include a second emergency contact name, address and phone number 2) The record for child #2 did not include the address for both emergency contacts listed. 3) The record for child #3 did not include a second emergency contact name, address and phone number and the information for the second parent to include the home address, home phone number, employment location and employment phone number.

Plan of Correction: The center director will have the parent fill out the forms tomorrow.

Standard #: 22VAC40-185-70-A
Description: Based on records review and staff interviews, the licensee did not ensue there was a complete record with all required information for all staff. Evidence: During the inspection on September 18, 2017, it was determined four out of the six staff did not have complete records; 1) The record for staff #6 did not include documentation that two or more references as to character and reputation as well as competency were checked before employment. 2) There were not records available for review for staff 3, #4 and #5 and the center director reported they began work on 9-5-2017.

Plan of Correction: The center director will have the staff fill out the forms.

Standard #: 22VAC40-185-240-A
Description: Based on a record review and staff interviews, the licensee did not ensure each staff member had been provided with in writing all the required information by the end of the first day of supervising children. Evidence: During the inspection on September 18, 2017, it was determined four out of six staff did not have documentation indicating the following information was given in writing to staff #1 (date of hire 8-28-2017), #3, 4, and #5. Staff #3, #4 and #5 began work on 9-5-2017 as reported by the center director. 1) Job responsibilities and to whom they report 2) The policies and procedures listed in the standards that relate to the staff member's responsibilities 3) The center's playground safety procedures unless the staff member will have no responsibility for playground activities or equipment 4) Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by 63.2-1509 of the Code of Virginia 5) Confidential treatment of personal information about children in care and their families

Plan of Correction: The center director will review the information.

Standard #: 22VAC40-185-240-B
Description: Based on a record review and staff interviews, the licensee did not ensure each staff member had been provided with in writing all the required information by the end of the first day of supervising children. Evidence: During the inspection on September 18, 2017, it was determined four out of six staff did not have documentation indicating the following information was given in writing to staff #1 (date of hire 8-28-2017), #3, 4, and #5. Staff #3, #4 and #5 began work on 9-5-2017 as reported by the center director. 1) Procedures to confirm absence of a child when the child is scheduled to arrive from another program or from an agency responsible for transporting the child to the center 2) Procedures for supervising a child who may arrive after scheduled classes or activities including field trips have begun 3) Procedures for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center 4) Procedures for action in case of lost or missing children, ill or injured children, medical emergencies and general emergencies 5) Policy for any administration of medication 6) Procedures for response to natural and man-made disasters

Plan of Correction: The center director will given them the information.

Standard #: 22VAC40-191-60-B
Description: Based on records review and staff interviews, the licensee did not ensue all employees or volunteers must not be employed or provide volunteer service until the agency has the persons completed sworn statement or affirmation. Evidence: During the inspection on September 18, 2017, it was determined five out of the six staff did not have completed sworn statements as listed below; 1) The record for #1 (date of hire 8-28-2017) and staff #6 (date of hire 9-13-2017 reported by program director) did not include a completed sworn statements as it did not answer the question "have you ever been the subject of a founded complaint of child abuse or neglect outside the state of Virginia?". 2) There were not sworn statements available for review for staff 3, #4 and #5 and the center director reported they began work on 9-5-2017.

Plan of Correction: The center director will have the staff sign it.
