5. The following information is required in transportation vehicles:
a. Emergency numbers as specified in 22VAC40-185-550 F and H;
b. The center's name, address, and phone number; and
c. A list of the names of the children being transported.
22 VAC 40-185-210. Program leader qualifications.
A. Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent, and meet one of the following:
1. Have one of the program director qualifications in 22VAC40-185-190;
2. Have an endorsement or bachelor's degree in a child-related field such as, but not limited to, elementary education, nursing, or recreation, from a college or university;
3. Have three months of programmatic experience and at least one of the following education backgrounds:
a. A one year early childhood certificate from a college or university that consists of at least 30 semester hours;
b. A child development credential by an organization listed in ? 63.2-1738 of the Code of Virginia;
c. A teaching diploma from an internationally or nationally recognized Montessori organization; or
4. Have six months of supervised programmatic experience.
a. Within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children, including but not limited to:
(1) Child development;
(2) Playground safety;
(3) Health and safety issues; and
(4) Preventing and reporting child abuse and neglect.
The licensing administrator and the licensing inspector conducted an announced mandated initial inspection at the facility on Thursday, September 14, 2017 from approximately 2:30 pm until 5:19 pm. During this inspection, the licensing administrator and the inspector obtained revised copies of the parent and staff handbooks for review, provided consultation, reviewed staff records, completed measurements of the cafeteria and gymnasium areas, and inspected the designated areas for care, playground areas, and transportation. The licensing administrator and the licensing inspector reviewed emergency supplies, and age appropriate materials and equipment. Based on the configurations from this visit the program will be issued a conditional license and will be expected to comply with all licensing standards. Standard #: 22VAC40-185-540-C Description: Based on review of the emergency supplies, the center did not ensure the first aid kit contained the minimum requirements. Evidence: The first aid kit on the van used as the backup vehicle for transporting children did not contain a second triangular bandage. Plan of Correction: Corrected on site
Standard #: 22VAC40-185-550-H Description: Based on observation of transportation, the center did not ensure the emergency documents for vehicles contained all of the requirements.
Evidence: The emergency information document for the bus used to transport children to and from the center did not contain local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pick-up/drop off of children to or from schools, etc.).
