An unannounced A Subsidy Health and Safety inspection was conducted on September 22, 2017 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on record review of children's files that have listed allergies, it was determined the program has not obtained a written allergy care plan that includes instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. Child #1, #2, and #3 all have food allergies listed by the parent. Child #3 has a parent letter stressing the allergy severity for 3 foods. 2. All 3 files do not include written instructions from a physician in the event of a suspected or confirmed allergic reaction. Plan of Correction: We will get Physician's notes on all files that contain recorded food allergies.
