Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection Friday, September 22, 2017 from approximately 10:45am to 1:30pm.

The following ratios were observed with a total of 24 children present during the inspection:
Infants - 4 children with 1 staff
Toddlers - 3 children with 1 staff
Twos - 6 children with 1 staff
Threes - 5 children with 1 staff
Fours & Fives - 6 children with 1 staff
School age children were not present during the inspection.

The children and staff were engaged in a variety of activities when the licensing inspector arrived. Infants were observed playing on the floor with their teachers and eating. The Toddlers and Twos were observed eating lunch. The Threes were observed having books read to them on tablets from the local library and eating lunch. The Fours and Fives were observed having books read to them as they discussed leaves. Children were also observed taking bathroom breaks, washing their hands, eating lunch and resting at naptime. Children bring their own snacks and lunches from homes. Food was labeled with the children?s names and dated. All areas of the center including classrooms, hallways, playgrounds, and bathrooms were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, injury prevention plan, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered but medication was not present at the facility during the inspection.

During the inspection, five children's records and five staff records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Description: Based on observation, the facility did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children.

During the inspection, the licensing inspector observed a container of sanitizing wipes sitting on a shelf in the Preschool classroom.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of five staff records, one staff member did not have the required completed background check within the required time frame.

During the inspection, the licensing inspector observed the record for Staff #3 (hire date 10/22/14) contained a criminal record history dated 08/21/14. Criminal record history checks are required to be updated at least every three years from the date on the most recent report.

Plan of Correction: We will send off an updated request form.
