An unannounced monitoring inspection was conducted on September 6th 2017 between 11:30am and 1:45pm. There were 37 children in the direct care of 7 staff members. Children were observed napping and staff were observed watching the children and organizing their classrooms. The facility was clean, organized, and offered plenty of age appropriate materials for children to utilize. Standard #: 22VAC40-185-130-B Description: Based on record review, 1 of 5 children's records reviewed did not have documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child #3, born 3/24/2016, last immunization documentation on file was dated 9/28/2016 Plan of Correction: Child`s parents were notified and immediately emailed us a copy of the current updated form which is now placed in the child`s file.
Standard #: 22VAC40-185-160-A Description: Based on record review , 1 of 5 staffing records reviewed did not have documentation of a tuberculosis screening submitted no later than 21 days after employment that was completed within 12 months prior to hire date. Evidence: Staff member was hired May 2017 and submitted a Tuberculosis screening that was dated 8/20/2015. Plan of Correction: Correction was completed on site employee was sent to get her tb done. Her new updated tb is filled away in her personal file.
